Eric,

You can try with the Web Sites listed below, also in my opinion, since =
helicopters cases are not rather frequently found in formal =
investigations and supposing your simulator is also intended to be used =
as an IFR trainer to, there is a lot of cases of commuter aircraft's =
accidents where "lack of CRM " is evident and the environment and type =
of operation and performances are most of the cases equivalent to =
helicopters in terms of speed, operating altitudes, Crew composition =
etc. so, those cases can be also considered to create scenarios for =
LOFT/MOST/LOS=20

Hi, everybody!
I am Eric Wahren at SAS Flight Academy, Stockholm, Sweden. We have =
recently
taken into operation a Bell 412/212 helicopter simulator. We'd like =
to add
to our present CRM course some cases about "lack of CRM induced =
incidents
and accidents in helicopter operations". Does anyone have =
information about
such cases and where I could find them?
Best regards,
Eric=20
