Dilhan and fellow pros
There is an excellent "time in type" and "flight experience" analysis
done by Yacavone, Borowsky, Bason, and Alcov of the US Navy Safety
Center in 1992. It was published in "Aviation, Space, and Environmental
Medicine", January 1992. (Vol 63:72-74) The article says that reprints
can be requested from David Yacovone, Head Aeromedical Division, US
Navy Safety Center, NAS Norfolk, VA (USA) 23511-5796. I don't know if
he is still there but someone should be able to find a reprint if you can't
get ahold of the journal.

Hi all !!

I am currently involved in analyzing an accident investigation for an
university subject. The accident is Piper PA-31-350 Chieftain (VH-WGI)
which
crashed in Launceston, Tasmania, Australia, on the 17 th of September
1993.

The BASI investigation report found the following factors contributed.
1. Minimal endorsement training and pilot experience on type
2. Inadequate operator supervision
3. pilot decision making influenced by noisy alcohol affected passengers

Can anyone please help me to get more info about the accident or
related
info on above factors

thanx

Dilhan
