I am currently involved in analyzing an accident investigation for an
university subject. The accident is Piper PA-31-350 Chieftain (VH-WGI) which
crashed in Launceston, Tasmania, Australia, on the 17 th of September 1993.
The BASI investigation report found the following factors contributed.
1. Minimal endorsement training and pilot experience on type
2. Inadequate operator supervision
3. pilot decision making influenced by noisy alcohol affected passengers
