Is it necessarily a "less than optimal relationship"?

Poor decisionmaking can happen due to a number of interpersonal factors.
I know a husband-wife team who fly often together and have a wonderful
reputation and rapport. I also know a father-daughter team who frequently
flew together with similar results. Father-son teams are also not
uncommon.

It all seems pretty individual. I'd hesitate to endorse a blanket
rejection of any particular situation, insisting that everybody always
fly with strangers. With good CRM, it shouldn't matter with whom we fly.
I thought *that* was the exercise in CRM. Optimally, a blanket policy
regarding flying with spouses, siblings, offspring, etc would be
completely unnecessary.
