Good Afternoon CRMers!

I located the official Colombian accident report of the Cali accident.
Peter Ladkin has converted it into web format and you can find it at:

http://www.rvs.uni-bielefeld.de/~ladkin/Incidents/Cali/calirep.html
(Text and Appendix A)

http://www.rvs.uni-bielefeld.de/~ladkin/Incidents/Cali/appendices.html
(App. B through F)

Additional information is available at:

Best regards,
Neil Krey

There are two documents you should refer to:
1. Bill Bulfer's comprehensive composite including transcripts and
static FMS data
2. Dave Simon's analysis of the multiple errors and deficiencies
involved, some 28 significant ones from wrong transponder code to
failure to retract speed brakes.

Both papers should be commonly available but I'll be glad to point if
requested.
