Human Factors in ETOPS

Dear CRMers :
It seems to me that ongoing ETOPS( Extended Range Operation With Two-Engine
Airplanes, ref. FAR AC 120-42A ) project only considers the reliability of
engines as a hardware. I am not sure how the probability of both engines
failure in flight due to human errors is being taken into account in ETOPS risk
assessment. Human errors associated with ETOPS include an erroneous shutdown by
flight crew of the wrong engine and same maintenance errors by a mechanic on
both engines, etc. I would appreciate any comment or advice from those who have
special attention on this issue or who have developed specific programs in
flight crew and/or maintenance CRM/LOFT training.
