I agree with Gordon that error management is the key issue. So here are some
questions:
1. How do we manage error?
2. Where do errors come from?
3. Do we have, or do we need, a theory/model of error management?
4. Would crews benifit from having a basic understanding of human DM or do we
just need more procedures?
5. How can error management be observed (by process or outcome)?
