One additional comment: many programs center around "core elements" as
topics, and these topics are generic to so many operations. Elements such as
"situational awareness, decision-making, communication, workload management,
etc." Few seem to put emphasis on an element which I take very seriously;
policy and regulations. In a very simplistic view, I truly believe that if
an aviation community had a clear and rational set of rules, which were
respected and followed by the members of that community, safe and efficient
flight ops would result. Many accident investigations contain the violation
of rules, usually without a justifiable reason. While it can be easily
argued that we can not regulate for every dynamic in the aviation operation,
I will propose that rules keep us safe, retain the experience of past
mistakes, and are the FIRST flag of a developing ERROR CHAIN.
