Hello everyone
My name is Tony Kern and I am new to the group, although I've been
aware of it for sometime now. I am currently a history professor at the
USAF Academy, but have been actively involved in military CRM curriculum
development, ground and inflight instruction, and evaluation for about ten
years. In my efforts over the years, I've developed (with considerable
help) the Air Force mandate (AFI 36-2243) and five-tiered training progam
now attempting to be institutionalized by the Air Force. In addition,
I've written several case studies like the B-52 crash at Fairchild AFB in
94 (Darker Shades of Blue), and more recently a CRM analysis of the Ron
Brown CT-43 crash (Unforced Errors). I've just published "Redefining
Airmanship" by McGraw-Hill, an attempt to get individuals to take
responsibility for learning human factors upon themselves -- instead of
waiting to be spoon fed from some training organization and I am trying to
kick out another book this summer detailing the phenomenon of poor flight
discipline, which leads me to my next point.
Enough on the introduction. I am working on a couple new projects
involving "flight discipline" and "rogue practitioners." I'm looking for
case studies that you may know of involving poor personal or
organizational flight discipline and/or incidents involving a "known"
rogue or renegade who somehow escapes detection and punishment until
something bad happens. If you might have any leads I would greatly
appreciate them.
