"Crew Resource Management: An Introductory Handbook"
by James E. Driskell and Richard J. Adams (1992)
Federal Aviation Administration (FAA)
Document No. DOT/FAA/RD-92/26
It is the newer (and watered down) version of:
"Aeronautical Decision Making: Cockpit Resource Management"
by Richard S. Jensen (1989)
Federal Aviation Administration (FAA)
Document No. DOT/FAA/PM-86/46
I have used both as texts for collegiate CRM programs and as a valuable
resource for airline CRM training. The Jensen book is much more
detailed, especially because it has 6 appendices which include the NTSB
synopsis (and transcripts) of UA - Portland, Air Florida - DC,
EA - Everglades, UA - Salt Lake City, Air Illinois - Carbondale, and
Western - Mexico City.
Unfortunately, you can no longer get Jensen's version from NTIS anymore.
I have a much used copy of both. I can't part with my originals, but I
will send you a copy of both. If anybody has a better copy, please pass
them on to Neil. These are very valuable resources and should be readily
available.
