Effect of the term 'Pilot Error' by media & investigators on future flight safety

Hi!

My name is Gary Pesavento and I've recently subscribed to this forum as
a result of my interest in Human Factors issues. I'm an A330 F/O with
Cathay Pacific, having spent the previous 13 years in the RAAF, flying
F/A-18s and instructing on the Macchi. I've also begun to facilitate on
some of Cathay's CRM course.

I'm currently studying towards a Grad. Dip. in Human Factors in Aviation
and am writing a short paper examining the above topic. (ie. the effect
upon accident prevention, of the term 'pilot error', when used by the
media and some accident investigation authorities when describing the
causal factors associated with accidents in which errors were made by
the aircrew.)

I'd be grateful for any thoughts on the subject and will happily give
credit where due. Thanks for your input.
