Kerry, this may be overkill for your purposes but I use "The Challenger Launch
Decision" by Vaughan and a four-hour guided discussion (which includes the CRM
Films video on Group Think) during my "Managing High Technology" Workshops for
the California Department of Justice and Florida Department of Law
Enforcement. I've also discussed doing something similar with FAA.

Since I integrate the subject of Group Think into a week-long workshop that
has four months of field projects in advance, the approach might not work as a
stand alone but I'd be happy to send you one of the Class Summary Handouts for
