Ms. Merritt,
My name is Lt. Michael Madron, and I am a C-130 navigator in the 41st
Airlift Squadron at Pope AFB, NC. I am also the Squadron CRM
Facilitator. Our Wing has been developing a CRM emphasis program, with
10 newly trained facilitators, and I am currently working on several
projects within my squadron. I would like to introduce the programs
using data from a unit climate assessment survey. I noticed in your
paper "Creating and Sustaining a Safety Culture: Some Practical
Strategies" that you mention the Flight Management Attitude
Questionaire, which sounds like what I am looking for. Could you provide
me with any info on how to obtain an effective questionaire for my unit?
If so, I would be greatly appreciative. Also, is the above mentioned
questionaire something which must be sent off to be analyzed for maximum
benefit, or could I do that?I hope you don't mind me asking for help and
advice on this issue. Although I've been a part of the CRM culture since
the beginning of my flying service, creating a program to teach it is
quite a different matter.I've got a million questions and ideas I'd like
to run by you, but for now, I guess the questionaire will have to tide
me over!

practicality of it, sat down to write a response, and read
CRMDEEN's(Greg Deen, Rayteon) response to you. Deciding that I couldn't
have said it better myself, I didn't try.
