Good morning, interesting discussion on the alleged Rogue pilot.....
the issue of liability keeps popping up. I'm a courseware developer and
facilitator of CRM at my airline. Currently in the 98 program we discuss Air
Ontario 1363 Dryden accident. The material is presented from the viewpoint of
layers of protection or defenses to assist in preventing incidents and
accident. One issue that most pilots focus on is the fact that the crew was on
their 6th day of operation////fatigue////. We chose this accident because of
the events currently taking place at our airline: the merger of airlines,
introduction of Jets, just to name a few. The issue of fatigue is never ending,
and seems to be an industry problem, the question to be asked is: after
training a crew to be technical proficient and clothed with the armor of CRM
who is liable when a captain makes a poor decision that leads to disaster,
because he/ she was fatigue. Was it poor CRM on the part of the crew?
