questions. The first is AC 120-59 Air Carrier Internal Evaluation
Programs. The second is AC 120-56 Air Carrier Voluntary Disclosure
Reporting Procedures. The third is AC 120-66 Aviation Safety Action
Programs. This last one may be found at:

Several airlines have developed such programs.
