One of the first "Airmail" inputs I read told the real challenge to have an
annonymous reporting system: effectiveness.
The report was from a crewmember on a C-5 aircraft who told of a
check-pilot who used improper and unauthorized maintenence proceedures to
retract thrust reversers which had not retracted after landing. While the
pilot in command had decided to cancel the mission and call maintenence, the
check-pilot, once he succeeded in his efforts, ordered the PIC to continue
the flight.
The author of the report said it all with "If you guys in the training
business don't do something with the dangerous crewmembers, then your CRM
program is not worth anything."
He's is absolutely correct: if the reporting system flags a problem, the
problem must be addressed, and the solution must be publicized. If the
audience thinks their voice will make a difference, they'll talk. Otherwise,
don't be surprised if you hear nothing, or get very few reports, as the
Airmail system has seen.
