Thanks for the reply.

Air 2000 operates a fleet of 14 B757 and 4 A320. We take delivery of our
first B767 in 1999. Our operation is world wide charter work.

The presentation is basically to explain the concept of the Error Chain and
then to show a working example demonstrating that the chain could have been
broken at a given point. It would be useful to show the errors could be
resolved from within the flight deck as the presentation is primarily for our
pilots.

Hello, Barry Forster, welcome to the group.
A couple of questions to help us help you: what sort of "Error Chain"
conclusions would you like your class to see? Some incidents show the problem
within the aircraft, some show that the aircraft take-off was near the final
event of the Error Chain, indicating that a mishap was going to happen as soon
as a "local trigger", as James Reason puts it, developed.
Perhaps a bit of insight into your organization could help, such as how
big, type aircraft, type mission, etc.
