Welcome Barry,
The "Error Chain" has been around for a few years. I delevoped a
presentation based upon the notion a variables in effect or not ineffect. And
that these variables are not static but are in fact fluidic in as much as they
change instant by instant. There is not a hole lot a crew can do if a
critical piece has departed the aircraft, like a wheel. This constitutes a
static situation. Human performance is never static.
It always changes and is suseptible to error in the changing.
