Error Chain

Hello, Barry Forster, welcome to the group.
A couple of questions to help us help you: what sort of "Error Chain"
conclusions would you like your class to see? Some incidents show the problem
within the aircraft, some show that the aircraft take-off was near the final
event of the Error Chain, indicating that a mishap was going to happen as soon
as a "local trigger", as James Reason puts it, developed.
Perhaps a bit of insight into your organization could help, such as how
big, type aircraft, type mission, etc.
