*** Valentine's day 1996 Remembered
St. Valentine's Day is a day normally associated with love and
relationships. But, for some this day has another meaning. It's
one of grief and anger. Three years ago today, the FAA's Aircraft
Maintenance Division, AFS-300, wrote their recommendations on
VJ6A465W, a.k.a., ValuJet Airlines. Almost three months later
ValuJet Flight 592 would crash into the Florida Everglades
killing all on board. The AFS-300 report of February 14, 1996
addressed a number of issues including the airline's Accident
and Incident History, Violation History, NASIP results, FAA
Surveillance History, and finally its Recommendations. The
violations numbered 46, since 1993. The NASIP reported a
total of 58 findings of which 43 were maintenance related.
The report closed with these four Recommendations:
1. Consideration should be given to an immediate FAR-121
re-certification of this airline.
2. The overall surveillance of the air carrier should be
increased in FY96.
3. The close out dead line for the NASIP inspection is
February 28, 1996. Every effort should be made to meet
this dead line with positive corrective action.
4. When a violation of the FARs are detected the inspector
should consider past enforcement history before
administrative corrective action is offered.
Most are familiar with the ValuJet case. Some have probably
followed, or even studied the case in great detail. However
it is not until one visits the web site of Flight 592 that
you find there is more to the story. It's a story with a
name, a face, and a family. Look at the photos of the
passengers and crew. Never forget ValuJet. Never forget
the crime. Never forget the Victims. Never forget the
Families. Never forget ValuJet Flight 592.
