Like several other members of this group, I read your account with both,
interest and concern. Not too many years ago I was taxiing out (as a
passenger) at DFW on my way to present a CRM workshop for corporate flyers.
Our taxi route took us right by a DC-10 off the runway in the dirt with
collapsed landing gear. Needless to say, it was a sad sight. Just the day
before, the first officer, flying the approach in stormy weather and
approaching minimums, elected to initiate a go-around presumably due in
part to an unstabilized approach (could be wrong about that). The captain
countermanded his decision to go around, took control of the aircraft and
landed - subsequently sliding off the runway.
Admittedly, I did not read the official report, so there may be
inconsistencies in my account of the incident, but I believe it is the
essence of what happened.
So I guess the question is, " When is CRM not CRM?" Maybe when it applies
only to the first officer.
Good luck with your program.
