To date, Pete, the consensus is... there is no consensus. Some have
postulated the event was the product of poor CRM while others have said it
was a case of silent agreement. Others have said it is the result of peer
pressure and not wanting to go against the grain. But, (and I may have
missed it) I have not seen a direct comment on this.
I believe it is not uncommon...for two guys to sit there and watch the thing
degrade. I guess the emphasis should be not so much on how it happened but
telling guys, "this can and does happen. If you are not comfortable with it,
SPEAK UP and say something specific."
