The discipline part begs many questions. What signals are sent to other
crews? Was there some clue the nav should have picked up on, hence the
marginal rating. Did anyone critque the pilots?

I personally think you mentioned one of the few items belonging CRM that
is frequently included in Check-list's currently in use, even before CRM
has been included in Company's training programs. The way we find it, is
the typical "Cross-check" required for same important actions ( I.E.
Altimeter setting crosscheck, HDG crosscheck or before any action on FFCL
during an in-flight engine shut-down "Eng. # 1 confirm...") . This is
essentially a powerful Feedback between pilot doing any action and his/her
fellow pilot not doing that, but certainly depending on it.

One big goal for us as CRM facilitators/Advisors is to get more and more
CRM derived actions in Check-list, this is one way to bring CRM onto the
pilot's daily duties doing that safer. Is a way to transform CRM in an STD
procedure. ( SOP)
