Miss set nav freq and / or course is an old story. The CRM part of the
process is the lack of positive feedback. I briefed you therefore it will
happen! Somewhere in the SOP there has to be a confirmation process : The
nav is tuned and identified on.., the inbound course is...

The discipline part begs many questions. What signals are sent to other
crews? Was there some clue the nav should have picked up on, hence the
marginal rating. Did anyone critque the pilots?
