Dan Maurino conducted a study two or three years ago in which he found that
Procedures were involved in Active Failures in 16 of 24 reports analysed,
and Procedural Deficiencies in 15 of 24 Latent Failures. Dan may have more
recent information on this significant area.

SOPs (or simply Standard Procedures, not to alienate our maintenance
engineering community) and the issues of non-adherence and over-adherence
are the subject of an Aeronautical Society conference in London later this
year (Sept. 24). I will send Neil a draft programme to post when the
