As Neil said, I have been working in the area for a long time and have
written three of the "Hazardous Attitude" manuals. I have hard copies of all
of them. Please let me know if I can be of help. The original five were:
Anti-authority
Impulsivity
Invulnerability
Macho
Resignation
They are outlined in my book with Stan Trollip "Human Factors for General
Aviation" (Jeppesen-Sanderson, 800-621-5377) and in my book, "Pilot Judgment
and Crew Resource Management" (Ashgate, 802-276-3162). Numerous examples are
offered in both books.
