The best documentation of the hazardous attitudes is found in the work
sponsored by FAA and Transport Canada a number of years ago on
Aeronautical Decision Making (ADM). I have paper copies of several of
the reports. If you have trouble locating them, contact me directly at
<neilkrey_at_mail.airmail.net>. Dick Jensen of Ohio State University can
also help, I'm sure. He has been deeply involved in the ADM area for
nearly two decades.
Another source is on the CRM Developers web site. Go to the Search page
at:
http://www.caar.db.erau.edu/crm/search/
and search on "hazardous attitudes" (include the quotes). This will
give you several documents which cover this topic at one level or
another.
