Dear Mr. Sakuma,
I have just read your article "O-157 and Human Error" in which I think you
make several important points. Truly we human in our race to create
technology to eradicate error give rise to new and unforeseen types of error.
We have certainly witnessed this with our glass cockpits and automated
systems. But what good sense you make with your point that if we could
develop systems that are "error tolerant," this may be more effective in
preventing error. Thank you for sharing the article. With your permission,
I will distribute it to my Aviation Safety and Human Factors classes next
semester.
