Tony Kern and Vince Moody both had some good ideas on the subject. Since the
Navy and the FAA have developed anonymous incident reporting systems, they
should have some pretty good experience as to how to make them work,
including the avoidance of "embarrassing" flag officers. Perhaps a DOD
system, modeled after the best aspect of the Navy and FAA/NASA programs would
do the trick. Sounds like a good project for a bright and shining star on the
Air Staff (Are you there, Ken?)
