I just read the most fascinating book on failed CRM and teamwork and
wanted to share it with everyone on the net. It illustrates complacency,
co-pilot syndrome, communication failure, lost situation awareness and
over reliance on experience instead of good judgment. So what's new, you
ask? In this case the "crew" was walking, not flying at 29,000 feet. Jon
Krakauer's book "Into Thin Air" is the story of the disaster on the slopes
of Mt Everest last year, and aviation human factors types will instantly
recognize many common themes from our business. It's a rippin' good read,
and might spice up your next training session!
