One day, during a full-motion simulator mission, the pilot realized he
was too low to clear the terrain in front and he rapidly pulled the yoke full
aft. This simulator is capable of 30 degree pitch, but the pilot only
commanded 20. The flight deck pitched up so quickly the navigator didn't
have time time grab onto something, and fell down. Several papers that were
laying on the top of the co-pilot's instument panel fell into the copilot's
lap. Shortly after that, the pin holding the pilot's seat in position
failed, and the seat slid backwards, cutting the engineer's leg which was in
the way. The pilot yelled for the co-pilot to take control, who was rapidly
clearing charts out of his lap.
It was an interesting crew debrief.
