There is a chapter in my book "Human Factors for General Aviation" (Trollip
and Jensen, 1991, Jeppesen Sanderson 800-621-5377) that does just that --
covers the basics. I have a more detailed description in my book, "Pilot
judgment and crew resource management" (Ashgate Press 802-276-3162).
